The spaces for the children to fill in include prompts like:
"How old is Dad?" "What makes Dad smile?" "What is Dad's favourite food?" "What does Dad do when you're not around?"
There is also a multiple choice quiz, which determines what kind of father you have, is he Mostly A's - Dad-letic, mostly B's - DaDIY, mostly C's - Dad-asterous or mostly D's - Super Duper Dad?
There is a tick list, where the child can tick all the things that dad is, including 'Funny', 'Clever', 'Cool', 'Brave' etc, and all the things Dad is good at, including 'Cuddling', 'Singing', Listening' etc.
There are also spaces to draw Dad and his family, Dad on holiday, Dad in his new clothes, Dad as a baby and a trophy for Dad.
